everywhere HIPAA protected web base EMR, during an online or in-person registration prior to your physical therapy evaluation.


3. How patients will consent to use of SMS
       Patients will agree and consent to receive SMS text messages from Motus Physical Therapy and Wellness via intake, patient will include their name, date and phone number and will sign and check a box to agree to receive text message.


4. How We Use Your Information and Types of SMS communication
       Provide Services: We will use your information to respond to your inquiries and account notifications and provide customer support. To Communicate with you, we may use contact information to send you important updates, account notifications, and promotional messages related to our services. You can opt-out of receiving messages from us at any time.
